Changes in libido, focus, or performance don't automatically mean hormones are the cause.

But those changes can be reasons to have a conversation with a medical professional and determine whether testing is appropriate.

What Does the Research Say About Testosterone Therapy?

For men who are appropriately diagnosed with hypogonadism, testosterone replacement therapy has been extensively studied.

One of the most significant recent studies is the TRAVERSE trial, published in The New England Journal of Medicine.

The randomized trial included 5,246 men between 45 and 80 years old who had symptoms of hypogonadism, two fasting testosterone measurements below 300 ng/dL, and either cardiovascular disease or an elevated cardiovascular risk.

Researchers found that testosterone therapy was noninferior to placebo for major adverse cardiovascular events during the study period. However, some adverse events—including atrial fibrillation, acute kidney injury, and pulmonary embolism—were observed more frequently in the testosterone group.

The takeaway isn't that every man should be on testosterone.

It's that TRT is medical therapy. It should be used for appropriately evaluated patients with proper medical oversight, consideration of potential benefits and risks, and ongoing monitoring.

What About Energy, Focus and Performance?

This is where it's important to separate marketing promises from medicine.

There isn't a single treatment that automatically creates more energy, sharper focus, better workouts, or improved performance for every man.

If testosterone deficiency is contributing to symptoms, treating appropriately diagnosed hypogonadism may help certain outcomes. Research cited in the TRAVERSE publication notes that controlled trials in older men have demonstrated benefits in areas including sexual function, bone mineral density, correction of unexplained anemia, and modest reductions in depressive symptoms.

But optimization starts with identifying the problem.
